(New Client Form #1) Intake Form
This form pertains to your health history and your present awareness of any pain or discomfort.
(New Client Form #2) General Liability Waiver & (Optional) ROI
This form includes a trauma-informed care liability waiver, along with an optional section that allows you to authorize communication with other providers or chosen members of your support circle. Sharing that information is completely up to you, and only what you’re comfortable with will be included. My goal is to support you in a way that feels safe, clear, and collaborative.
(New Client Form #3) Intended Health and Wellbeing Form
This form is to help me better understand your needs, intentions, and goals for our time together. It also offers a gentle invitation to begin a personal agreement between you and your body, one that supports safety, presence, and steadiness throughout our sessions.
